The first diagnosing visit. You will receive and evaluation, therapy diagnosis and anticipated plan of care to get you on a path to feeling better. Typically run 45-60 min
This entails multimodal interventions of your plan, directed by a physical therapist. Typically run 45-60 min
The goal of this service is general wellness and doesn't serve as a medically necessary service. These appointments are 30 minutes and require some pre-treatment intake forms/health history for your safety.
The above prices are for self-pay patients and include the price list for insurances we am not yet contracted as in-network status with. If I am in-network with your insurance we are contracted and must submit claims to your insurance carrier if you choose to disclose your personal health information of your insurance carrier. The No Surprises Act, effective from 2022, requires therapists to provide self-pay clients with a Good Faith Estimate (GFE) outlining expected treatment costs to avoid unexpected bills.
